Vaccine Info for 7th Grade Students

Students entering 7th grade are required to have one dose of Tdap (tetanus, diphtheria, pertussis) vaccine and one dose of meningococcal conjugate vaccine for individuals who have not already received those vaccines. Documentation of these vaccines must be provided to the school; compliance will be audited.

Please be sure to request a copy of your student's vaccines and email them to the school.

Please email any questions to Nurse Wilkes @ terri.wilkes@nhcs.net. Doctors, pharmacies, and vaccine clinics do not forward copies routinely. Please also keep a copy for your student's health record at home.
